Policy
Appointments
are required. We are sorry that we cannot accommodate
walk in appointments. Please give 24 notice if you are
not able to keep a scheduled appointment.
Please
advise us if there has been any change in your address,
phone number or insurance coverage since your last appointment.
Payment
is expected when services are rendered unless prior
financial arrangements have been made.
HMO
SUBSCRIBERS:
- You
must have a referral from your primary care physician.
If you do not have a referral for your office visit,
you may be asked to reschedule your appointment.
- Insurance
cards and copays are required at time of service.
As
a courtesy to our patients, we will bill any commercial
insurance carrier assigned, but the patient remains
financially responsible for all services furnished.
Any outstanding balance after 60 days from the date
of invoice is considered in default.
Workers'
Comp patients must bring all applicable insurance information
to initial appointment including:

address and phone number
- Adjuster
name and phone number
- Claim
number
